The information will be stored in protected physical and electronic media. It will be collected by the doctor in face-to-face consultation with the owner of the data. If the patient is a minor, they must be accompanied and authorized by their parents or guardians.

 

The information collected is intended to provide high quality medical care, preparation of medical and surgical events, control, monitoring and medical review of the patient. In no case will the information obtained be disclosed, transferred or shared with third parties without the express authorization of the owner. The aforementioned treatments can be accessed, rectified, opposed and, where appropriate, canceled by the owner by going to the doctor's office and a response will be given under the criteria of time and form of the Regulations and the Federal Law on Protection of Personal Data Held by Private Parties.
